-endocarditis

5-Side effect:
-diarrhoea
-indigestion
-occasional rash
- erythematous rash
6-Contraindication:
It should not be administered in patients hypersensitive to penicillin
7-Precautions:
-during treatment with high doses an adequate fluid intake and urinary output must be maintained
-indwelling urinary catheters should be regularly checked for potency since high urinary concentration of some drug may precipitate out of solution
8-Dose:
*general recommendation:
Adult:
-oral / IM injection: 250 – 500 mg every 8 hours
-IV injection / infusion: 500 mg – 1 g every 4 – 6 hours
Children:
- Oral / IM injection: 125 mg three times daily increasing to 250 mg in severe infection.
Amoxil pediatric drops are recommended for children under 6 months: 35 – 100 mg/kg/day.
-IV injection / infusion: up to 100mg/ kg / day in divided doses every 6 hours by infusion over 30 minutes
*specific dosage recommendation:
-Typhoid and Paratyphoid:
Adults…….. 4 gm daily for 14 – 21 days
Children………100 mg / kg / day in divided doses for 14 – 21 days
-Typhoid carrier states …… 3 – 4 gm daily for a minimum of one month
-gonorrhea ……….. Single dose of 3 gm plus 1 gm probenecid
-uncomplicated urinary tract infection……… a single dose of 3 gm
-meningitis:
Adult ……. 150 mg / kg / day in equally divided doses
Children …..100 – 150 mg / kg / day in equally divided doses
-syphilis …….. 250 mg every 6 hours for 4 weeks up to 5 months
-sever or recurrent lower respiratory tract infection ……. 3 gm twice daily
-dental abscess …….. Two 3 gm doses with 8 hours between doses
-otitis media …… 750 mg twice a day used in children from 3 – 10 years
9-Over dose:
-Since it is a penicillin over dose problems are unlikely to be encountered
-Will produce very high urinary concentration
-Crystalluria is possible
- The antibiotic is removed by heamodialysis
10-Storage:
-store in dry place below 25 C
-once dispensed the syrups should be used in 7 days
-keep out of reach of children
